NOW ACCEPTING PASSPORT APPOINTMENTS
The City of Los Angeles Office of the City Clerk is pleased to announce that it now accepts DS-11 forms for NEW passport applications on behalf of the U.S. Department of State BY APPOINTMENT ONLY.
Appointments are available Monday through Friday from 8:00 a.m. - 12:00 p.m. and 1:00 p.m. - 3:00 p.m.
Requirements to Submit an Application with the City Clerk’s office
At least one of the following must be true:
- You are applying for your first U.S. passport
- You are under age 16
- Your previous U.S. passport was issued when you were under age 16
- Your previous U.S. passport was lost, stolen, or damaged
- Your previous U.S. passport was issued more than 15 years ago
Must bring own passport photo. See photo requirements below.

*We do not accept renewals using form DS-82. (Go to travel.state.gov for information)

Fees (both Fee #1 and Fee #2 must be paid during appointment)
Payable to the City of Los Angeles
Execution fee: $35 per application
